BoneSense · Fracture Liaison Service platform

Catch the next fracture
before it happens.

Bone health, connected.

A digital fracture liaison service that identifies fragility-fracture patients, applies risk stratification inside the clinical pathway, and populates FLS-DB reporting as a by-product of routine care.

Built for FLS coordinators, nurses and consultants. MHRA-registered Class I medical device software.

The by-product of routine care

Every fracture becomes a data point that protects the next patient.

BoneSense captures FLS-DB KPIs and FRAX-based risk as care is delivered — feeding the national picture without extra work at the bedside.

The problem

A fracture is a warning. Most are missed.

A first fragility fracture is the clearest signal that another is coming. Yet most people who break a bone after 50 are never assessed for the underlying bone health, and access to fracture liaison services remains uneven worldwide.

37M+

fragility fractures occur worldwide each year in people over 55 — around 70 every minute. IOF

~80%

of high-risk patients are neither identified nor treated for the osteoporosis behind their fracture. IOF

1 in 4

people die within a year of a hip fracture; in the UK, hip fractures alone account for around one million bed-days a year. ROS

What BoneSense does

One pathway, from identification to follow-up.

BoneSense brings the whole fracture liaison service into a single clinical record: finding patients, stratifying risk, and handling audit and follow-up as part of routine care.

01
Find

Automatic case finding

Identifies eligible patients automatically, so far fewer are missed than with manual searching.

02
Assess

Risk stratification in the pathway

Applies a validated fracture risk stratification tool inside the record, so the assessment that guides care is captured once, without separate re-entry.

03
Report

Audit built in

Captures the national FLS-DB key performance indicators as care is delivered, with a live view of performance ahead of the submission deadline.

04
Follow up

Structured monitoring

Automated follow-up, scan reminders, treatment-adherence tracking and GP letters, with quality-improvement reporting aligned to the RCP framework.

05
Medication

Bone-health medication tracking

Track the anti-osteoporosis and bone-health medication prescribed to fracture patients — adherence, review dates and outstanding actions in one place.

Patient involvement

Bone health, in the patient's hands.

Prevention only works when patients stay engaged long after they leave hospital. BoneSense puts a mobile-first experience at the centre of the pathway, so patients can take an active part in their own care rather than waiting for the next appointment.

  • Follow-up questionnaires and reminders that patients complete on their phone, wherever they are
  • Clear, accessible information about their fracture, their risk and their treatment
  • Medication and adherence prompts that support people to stay on treatment
  • A simple way to raise a concern between visits, so problems surface earlier

In partnership with the Royal Osteoporosis Society, BoneSense embeds trusted patient education into the pathway, giving people consistent, expert guidance on protecting their bone health.

The evidence base

What fracture liaison services achieve.

The case for BoneSense rests on the published evidence for the FLS model of care. These findings describe fracture liaison services in general, not this product; BoneSense is software designed to help teams deliver that model consistently.

01

A systematic review and meta-analysis of 74 controlled studies found FLS programmes significantly increased bone-density testing, treatment initiation and adherence, and reduced refracture and mortality rates.

02

A 2021 systematic review and meta-analysis reported a significantly lower probability of subsequent fracture among patients managed through an FLS compared with no FLS.

03

The FLS-DB report found the proportion of high-risk patients started on anti-osteoporosis treatment within 16 weeks rose from 30.0% (2022) to 35.4% (2023), and identifies this as the national priority for improvement.
